Consumer Rights & Contact Information
In Accordance with House Bill 4224 (89th Regular Session) and Texas Health & Safety Code §181.105
C3 is committed to protecting the privacy and confidentiality of patient health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA), applicable Texas law, and professional ethical standards. The information below is provided to assist clients and the public with:
- Requesting copies of protected health information (PHI)
- Contacting the applicable licensing authority
- Filing a consumer complaint
Requesting Your Health Care Records
Current and former patients may request copies of their health care records by completing the Health Care Records Request Form below.

Records Request Fee
C3 charges a standard flat rate of $75 for most health care records requests. This fee covers copying and preparation of standard records. Additional fees may apply if your request is unusually large, requires extensive labor, or involves special delivery methods (such as CD or secure courier). Any additional fees will be communicated to you before your request is processed.
To Avoid Delays, Please Include:
- Full name and date of birth
- Phone number and/or email address
- Description of the records requested (entire record or specific dates)
- Preferred delivery method (secure email or in-person pickup)
- Signature and date
If requesting records on behalf of another individual, documentation demonstrating legal authority (e.g., guardianship or medical power of attorney) is required. Identity verification may be required to protect patient privacy and confidentiality.

Texas Behavioral Health Executive Council (BHEC)
C3 is regulated by the Texas Behavioral Health Executive Council (BHEC), the licensing authority for Licensed Professional Counselors (LPCs). Services are provided in accordance with the Texas Administrative Code, the Texas Behavioral Health Executive Council Rules, and the American Counseling Association (ACA) Code of Ethics.
